Testosterone Syringes and Needles | Which Ones Do You Need?
This article is for education only and doesn't replace advice from your doctor. Always follow your prescriber's instructions for needle gauge, length, and syringe size. Your pharmacist can also help you pick the right supplies for your prescription.
If you're new to injecting testosterone, the supply aisle is a wall of numbers. Gauge, length, slip tip, luer-lock, draw needle, inject needle. Nobody explains what any of it means. So here's the rundown: what the numbers mean, which needle size to use for thigh and glute shots, how SubQ differs from IM, and what to buy.
Syringe vs. needle
The syringe is the plastic barrel with the measurement markings and the plunger. The needle is the thin metal part that attaches to its tip and goes into your skin. You can buy them together or separately. Many people use two needles per shot: a bigger draw needle to pull oil from the vial, then a smaller inject needle for the shot.
What size syringe for testosterone?
A 1 mL syringe works for most testosterone doses. It's precise, easy to handle, and right for typical weekly or twice-weekly SubQ doses. A 3 mL syringe works better for larger IM doses, or if you draw from a multi-dose vial and dose less often (every other week or monthly).
| Syringe | Best for | Notes |
|---|---|---|
| 1 mL | SubQ, small weekly doses (0.1–0.5 mL) | More precise markings. The Inject-Ease needs a 1 mL Slip Tip. |
| 3 mL | IM, larger or less frequent doses | Most common at pharmacies. Better for doses over 0.5 mL. |
Needle gauge: what the number means
Gauge is how thick the needle is, and it runs backwards: a higher number means a thinner needle. A 27g is much finer than a 21g.
| Gauge | Thickness | Use |
|---|---|---|
| 18–21g | Thick | Drawing oil from the vial only. Never inject with these. |
| 22–23g | Medium | Standard IM shots, thigh or glute |
| 25g | Fine | IM or SubQ. Less discomfort, slightly slower flow |
| 27g | Very fine | SubQ only. Minimal pain |
For most IM shots, 23–25g is the sweet spot. It's thin enough to be comfortable and thick enough for oil-based T to flow. Going thinner than 25g for IM can make the shot painfully slow.
What size needle for a testosterone shot in the thigh?
For IM in the outer thigh (vastus lateralis): 23–25 gauge, 1 inch. A 1" needle reaches the muscle for most people. If you have more body fat there, your prescriber may suggest 1.5". Always follow your clinic.
- Inject into the outer, upper part of the thigh, not the inner thigh or the very top near the hip.
- Sitting with your leg relaxed makes this spot much easier to reach on your own.
- Switch between left and right each shot so each side can recover.
What size needle for a testosterone shot in the glute?
For IM in the glute (gluteus medius): 23–25 gauge, 1–1.5 inches. The glute usually has more tissue between skin and muscle, so 1.5" is the most common length. A 1" may be enough if you're leaner. Your prescriber will tell you what's right for your body.
- Inject into the upper outer quarter of the glute, not the center or lower part.
- It's hard to reach on your own, so many people stick to the thigh for solo shots.
- If you're standing, shift your weight to the other leg. It relaxes the muscle and hurts less.
SubQ needle sizing
SubQ shots go into the fat just under your skin, not into muscle, so they use a shorter, finer needle: 25–27 gauge, 5/8 inch. Common spots are the belly, outer thigh, and upper arm. A lot of people find SubQ easier to do themselves and less sore.
SubQ is widely used in gender-affirming HRT. UCSF's masculinizing hormone guidelines include it, and many clinics now prescribe it by default. If IM is hard or painful for you, ask your prescriber whether SubQ works for your formulation.
Needle anxiety? The Inject-Ease is a spring-loaded auto-injector for SubQ only. It uses a 25–26g, 5/8" needle with a 1 mL Slip Tip syringe, and it puts the needle in for you when you press the button.
Draw needle vs. inject needle
Most beginners miss this one. The draw needle (18–21g) pulls the thick oil out of the vial fast. Then you swap it for a fresh inject needle (23–25g for IM, 25–27g for SubQ) for the shot.
Why swap? Pushing through the rubber vial stopper dulls the needle. A dull needle hurts more and does more tissue damage. Some clinics skip the swap and use one needle. That's fine too, just a little less comfortable.
Slip tip vs. luer-lock
These are the two ways a needle attaches to a syringe. A slip tip has a smooth, tapered tip, and the needle slides on and is held by friction. It's quick, and it's what the Inject-Ease needs. A luer-lock has a ridged, threaded tip, and the needle screws on, so it's held more firmly. People tend to prefer it for IM.
Not sure which you have? Look at the tip of the barrel, check the product listing, or ask your pharmacist.

Safety basics
- One needle, one shot. Never reuse needles. They go dull after one use, which means more pain and a higher risk of infection.
- Never recap needles. Put them in a sharps container (most pharmacies have them).
- Rotate spots. Injecting the same place builds scar tissue, so your body absorbs less and it hurts more.
- Check the expiration dates on needles and syringes. They may not stay sterile past the printed date.
- Wipe the spot with alcohol, then let it dry fully (about 30 seconds) before the needle goes in.
- Keep your kit organized. Scrambling for supplies on shot day means more stress and more mistakes. The TGS Shot Kit is a hard case that holds your vial, syringes, and the rest.
FAQ
Some people use insulin syringes (U-100, 27–31g, 1/2") for SubQ T. They're very fine and barely hurt, but the small barrel makes drawing oil very slow. If your dose is small and you're doing SubQ, it can work. Check with your prescriber first, and make sure they're U-100, not U-40, which are for pets.
Use a sharps container. Most pharmacies have them, often free or cheap. Never put loose needles in the trash or recycling. When it's full, most areas have drop-off spots at pharmacies, clinics, or sharps waste sites. Check your local rules.
Getting your supplies dialed in is one of the easiest ways to make shot day less of a thing. The right gauge means less pain, and the draw/inject swap takes ten seconds.
