What's the deal with Oral Testosterone?

Testosterone comes as injections, gels, patches, pellets, and capsules you swallow. More people in our community have been asking about oral T lately, so here's what it is, how well it works, and what to ask your provider before you bring it up.

What is oral testosterone?

Oral T usually means testosterone undecanoate. It's absorbed through the lymphatic system instead of the liver, which avoids the liver damage that made older oral T unsafe.

Brand names you might see:

• Jatenzo
• Tlando
• Kyzatrex
• Andriol / Restandol (outside the U.S.)

These are prescription meds, and your doctor needs to monitor you regularly while you take them.

Can trans masc people get it?

Yes, with limits. In the U.S., oral T is FDA-approved only for hypogonadism in cis men, so prescribing it for gender-affirming care is off-label. Many doctors can prescribe it, but not all will, because:

• There's limited research in trans people.
• It can raise blood pressure.
• Insurance often won't cover it, and it can be expensive.
• Your levels may swing more than they do with injections or gel.

So it's not out of reach, but it isn't the usual first choice for masculinizing HRT.

How well does oral T work?

Taken the right way, with a meal that has at least ~20 grams of fat, oral T can bring your levels into the typical male reference range. That can support:

• Voice deepening
• Facial and body hair growth
• Fat redistribution
• Higher libido
• Clitoral growth
• Muscle development

Absorption varies more than with injections or gel, though, so your blood levels may rise and dip more.

Why isn't it the standard?

Most providers prefer injections or gel because they're better studied and more predictable. Oral T's main drawbacks:

• Heart health. It can raise blood pressure, which raises cardiovascular risk. It carries an FDA black box warning for this.
• Cost. Depending on insurance, it can run from hundreds to over $1,000 a month.
• Less steady levels. Diet, timing, and metabolism all change how much you absorb.
• Less research. There are fewer long-term studies of oral T in trans men, trans masc, and non-binary people.

Are there upsides?

For some people, yes. It's discreet, there are no needles or gel, it's easy to take daily, and it clears your system quickly if you stop. If a health issue rules out injections, or needles make you really anxious, it may be worth talking over with your doctor.

Questions to ask your provider

• Is oral T safe for me, given my blood pressure and health history?
• How often will I need lab work?
• Will my insurance cover it?
• Could it help if my current method gives me mood swings or uneven levels?
• What dose makes sense for gender-affirming therapy?

Oral testosterone is available, it works, and some trans masc folks use it. In the U.S., though, it's not the most common or recommended form of gender-affirming HRT.
